- The campaign runs now through November 30th.
- The appointment must be booked and completed by November 30.
- The official campaign entry form must also be submitted by November 30.
- Completing only the appointment or only the form does not qualify as an entry.
- One patient will be selected after the campaign ends.
- The selected patient will receive $500 toward Big City Optical purchases.
- Big City Optical will donate $500 to the participating nonprofit selected by the patient.
